This form is used to submit Safety Conversations pertaining to the usage of SWA, Near-Miss, Good Catches or simple safety suggestions. This form can be used anonymously to report a finding and protect your identity. Using the form this way protects your identity and delivers the message to the Safety committee directly. This form is also used by leaders to report mandated Safety Conversations and the Foreman/Reporting Party field must be filled in order to receive credit for your submission in this manner.
